Client Contract

My fee is $125.00 per session, payable at the beginning of each session.  I do not take insurance, and can give you a receipt if you wish to bill your insurance company on your own. I am considered an out-of-network provider. Payment is in the form of cash, check, PayPal, Venmo, Visa, or MasterCard.

I provide therapy primarily by phone, but also offer video sessions when requested.  All clients using this modality must read and submit the Client Intake Form and Declaration of Understanding prior to the beginning of our work. Payment is due prior to the therapy session.
